Assessments Program

The Care Solutions Group Assessments Program includes a comprehensive, three-tiered risk-adjusted evaluation that accurately reflects the health of the patient.

Each evaluation captures each patient’s health status and includes a review of the following areas:

• Past medical history

• Functional activities of daily living

• Cognitive and behavioral health

• Medication review and reconciliation

• PCP review and follow-up

• Social and family support

• Recommended services
