To screen and complete our due diligence, Care Solutions Group (CSG) has provided the following timeline for our application and approval process:

• Issue Request for Proposal (RFP) to prospective applicant

• Prospective applicant to submit RFP response

• CSG to complete review of submitted RFP (two weeks required)

• CSG and applicant to conduct roundtable discussion(s) to address outstanding questions / concerns (one week required)

• CSG to issue decision on applicant (one week required)

• Applicant to review/sign Member Agreement to become Member of CSG (one week required)

• Member to attend initial meeting with CSG Members
