December 2019: Care Solutions Group and its affiliate, Premier Physician Care, completed 2019 with its best performing year for hospital readmission rates – the practice concluded the year with a 6.4% 30-day readmission rate and a 12.8% 90-day readmission rate.
February 2020: Care Solutions Group, in collaboration with its provider group including its affiliate, Premier Physician Care, commenced exploratory discussions with a chain of skilled nursing facilities in the Tri-County Metropolitan Detroit area to connect our patient population to nearby facilities for rehabilitation services and seamlessly return the patient to their home.
March 2020: Consistent with guidelines published by CMS and to eliminate exposure to the most vulnerable, Premier Physicians Care launched its Telehealth Program on March 29, 2020.
May 2020: Premier conducted its first long-distance telehealth visits with residents in Grand Rapids, Petoskey and Spring Lake, Michigan.
