Care Solutions Group, in collaboration with a chain of skilled nursing facilities, has been measuring its 30-day readmission rates for discharged patients from multiple rehabilitation communities. Since our collaboration launched in 2020, our cumulative 30-day readmission rate totaled 10.8%, significantly lower than the national average of between 15% and 20%. The key factors in lowering this rate to the present level are attention to detail, consistent follow-through and superb execution.
